Get an RX (Referral)- obtain from primary medical provider
Must include:
- "Evaluate and Treat" for speech, occupational and/or physical therapy.
- ICD-10 diagnostic code/s
- Signature of doctor (MD), pysician's assistant (PA) or nurse practicioner (NP).

Access Patient Portal
- Complete Service Request form.
- Receive email link for Patient Portal registration.
- Login to your secure Patient Portal and complete & submit Intake Forms.
*Patient Portal registration does not constitute a confirmed appointment. Our office will contact you for scheduling an initial appointment following receipt of all required documents.
